  • Conduct benchmark assessment and develop case plans for all 100% of the beneficiaries (caregivers & VCAs) and provide a service at a minimum of once per quarter.
  • Conduct monthly household visits to at least 100 households ensuring all appropriate forms are filled in and up to date, ensuring that casefiles/case plans/tablets are kept in a safe and secure place.
  • Ensure 100% of VCAs in the my care eligible for testing with unknown HIV status will be tested and have a documented result (facilitate the know your child status).
  • Ensure services are provided to the household in all the four domains, health, stable, schooled and safe.
